Immunizations Records Requests
Ƶ retains immunization compliance records indefinitely. Current and former students may request a copy of their immunization records by completing the . Requests are usually processed within 30 days.
Your medical information will not be released without your permission except in life-threatening emergencies, or unless required by federal, state, or local law(s). In the event of telephone calls from parents about your medical care Ƶ Health Services or the Student Health Insurance and Immunization offices are unable to release any medical information to your parents without your explicit verbal or written permission if you are 18 years of age or older. A “blanket” authorization form is not allowed for students to sign in case of what you might want to release to a third party now, you might not want to release at a later date. A release form will need to be submitted and signed each time you want new information released from your records.
During times of the transfer of medical care to another health care institution or organization is necessary. In order for medical information to be released, a written release must be signed by the requesting student.

The Immunizations Records Request Form is only for current and former students seeking past immunization records that were submitted during the matriculation process to the university. The Immunization Records Request Form is not available for incoming, matriculating students for the upcoming semester.
Please note: Recently accepted students for the upcoming semester, (INCOMING STUDENTS) will need to submit vaccine documentation to the for verification to be in compliance for the Immunization Requirement as part of your matriculation process for attendance to the university. Vaccine records from your physican, healthcare agency or pharmacy in which your vaccines were administered will need to be submitted by July 1. Ƶ does not have vaccine records for recently accepted, incoming students. Please upload your vaccine documents on the Immunization Compliance Verfication Portal.
